Preventive exams are an important way for you to stay healthy in the long run. Meeting with your doctor every year helps you identify any areas of concern before they develop into more serious (and costly) conditions.
Similar to last year, you can save money by taking action towards better health! You will receive a discount on your monthly medical premium, if you and your spouse/domestic partner both get a preventive exam.
Your results are never shared with your employer. Your incentive is based solely on participation, and does not depend on the exam results.
There are 2 options this year, that will allow you to take advantage of the savings –
1. Option 1: Schedule a preventive physical exam with your doctor
If you or your spouse/domestic partner already had a preventive exam this year, results from that screening can be used.
2. Option 2: We are continuing to offer employees a way to schedule a Virtual Check-up* through Catapult. See attached and below for more details. If you choose this option, you do not need to complete the Vivacity Exam Form. A list of those who have completed the at-home exam will be provided for use in giving the wellness incentive. (Only available for Premera Members)
3. If you choose to make an appointment with your doctor, use the Preventive Exam Form (available through HR) and complete the required sections. Please take the form to your appointment for your doctor to complete and sign
Provide your email address on the form and you will receive an email confirmation within a week of your form being processed.
4. By November 30, 2021 send the form and lab results (if applicable) to our wellness partner, Vivacity. You or your doctor’s office can send the form via:

Important note: Screening for any values other than those listed in Section 3 of the form may be subject to deductible, copay, and coinsurance.
* The VirtualCheckup™ includes a blood pressure monitor (yours to keep), a tape measure, a blood pressure log, and everything you need to collect a small blood sample using a spring-loaded lancet for a finger stick, easy-to-understand instructions, and a video consultation with a board-certified Nurse Practitioner. Patient information is strictly confidential – your individual results will not be shared with your employer. Fasting is recommended but not required. (Participation in the virtual health checkup is not available for women who are pregnant, have been pregnant within the past 6 weeks, or have a history of double mastectomy with bilateral lymph node removal. If any of these pertain to you, please continue routine care with your healthcare provider.)
